Community Nurses

The district nurses provide general nursing care in the home to patients who are unable to get to the surgery, or for whom a home visit is more appropriate. The district nurses can be contacted on 01264 835372.

The Proactive Team works alongside your practice. The teams are made up of Care Coordinators and Proactive Care Nurses who work pro-actively to help you improve your quality of life and healthcare experience. It is aimed at patients over 70 and you can self-refer into this service or you may be referred into the service by your own GP, another agency or healthcare supplier. The Proactive Care Team are able to provide information on local health and social groups: Befriending Services Social Community Groups Dementia Support Services Carer Support Services.

The Proactive Care Nurse is able to carry out an assessment and refer if appropriate to other services, such as: Community Nursing Teams Occupational Therapy Continence Service Tissue Viability Service Physiotherapy Community Dental Services Chiropody Service Adult Social Services.
